COMMUNITY NAVIGATOR
The purpose of the Community Navigator position is to link individuals with mental illness to supports and services in the community, to help them navigate the income support system, and to assist in personal goal-setting and the development of expanded personal support networks. The Community Navigator works closely with the Outreach Worker from the branch or other agencies to select clients currently on Income Assistance who may be eligible for higher levels of income support such as the PPMB (Persons with Persistent Multiple Barriers) or PWD (Persons with Disability) designations through the Ministry of Housing and Social Development. The Community Navigator assists clients in getting connected with a diverse range of community groups within and outside the formal mental health system in order to ease access to supports and services. The main effort is to link people to existing community supports that can help them meet a range of their needs including and beyond income.
